I hereby grant consent to AbleNet, Inc., its parents, subsidiaries, related entities, licensees, agents, officers, employees and their successors and assigns (collectively referred to herein as “AbleNet”) to capture and use my name and the following (my name and all bulleted items, collectively, “My Likeness”):
- images of me
- video of me
- written comments, statements, or testimonials given by me
- sound recordings of me
I further grant AbleNet permission to use, and to grant a license to others to use, My Likeness, for educational and promotional purposes including, but not limited to: use in internal and external communications; publication and republication of it in print and/or any form of digital media; making it available on the Internet, including but not limited to, use on website(s) and in any and all social media; and use of it for purposes of advertising, promotion and publicity relating to the products, services and business of AbleNet. AbleNet will not make any derogatory use of My Likeness.
I have not granted to any third party any license to use My Likeness in a manner that will conflict with the rights granted to AbleNet herein.
I acknowledge that I will not have the opportunity to examine, edit or approve the manner, completed product, or method in which My Likeness is used and that AbleNet has no obligation to use My Likeness. AbleNet will own all property rights, including intellectual property rights, in the material including My Likeness.
I grant this consent and release as a voluntary contribution to AbleNet and for other good and valuable consideration, receipt of which is hereby acknowledged.
I hereby waive and release AbleNet from all liability for claims and demands arising out of or relating to the use of My Likeness (including, without limitation, claims based on invasion of privacy, defamation or right of publicity.
I warrant that I own the copyright in My Likeness and that I have the permission to grant AbleNet the right to use and reproduce My Likeness in accordance with the terms herein. In the event a third party owns rights to My Likeness, I warrant I have obtained the necessary consents, licenses, or releases from third parties to grant the rights granted hereunder
I am over 18 years of age, and I have the right to make this agreement. BY PROVIDING MY ELECTRONIC SIGNATURE BELOW, I ACKNOWLEDGE THAT I HAVE READ THE ABOVE AUTHORIZATION, RELEASE AND AGREEMENT AND FULLY UNDERSTAND THE CONTENTS THEREOF. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY. This agreement shall be binding upon me and my heirs, legal representatives, and assigns. I understand that my electronic signature and acknowledgment have the same legal force and effect as a manual signature. AbleNet will provide a signed copy of this agreement upon my request.
